(Please note we can only accept applications of people who have family or direct ties to the Midwest Region and are known by Management or Directors who can confirm your Aboriginality through the information provided. If we cannot confirm your Aboriginality through the information provided in this application, it will be declined).

If you have said no to any of the above, please do not complete the remainder of the form as it will most likely be rejected, as it is a legal requirement for approval applicants are required to agree to all the above.
